Menopause-associated weight gain is driven by the decline in circulating oestradiol, which alters energy homeostasis through multiple mechanisms: reduced resting energy expenditure, a shift toward central (visceral) adiposity due to changes in lipoprotein lipase activity and adipocyte oestrogen receptor signalling, increased insulin resistance, and altered appetite regulation via changes in leptin sensitivity and ghrelin dynamics. The average weight gain during the menopausal transition is 2-5 kg, with a disproportionate increase in visceral adipose tissue that elevates cardiometabolic risk independent of total body weight.
